I don't know to which extent such metadata is reflected in file naming scheme of in other metadata in file system as some modern file system provide options to add metadata too. That's the reason why you want to provide special protection in order be compliant to HIPAA and possibly other regulations. To me it seems like the best bet is to encrypt the data at rest on a file share limited to only admin who has a decryption key.
